PRIVACY:
Optometrists Association Victoria respects your right
to privacy. We collect the names, addresses, email
addresses and telephone and fax numbers of SRC
delegates for the purpose of contacting delegates
when necessary, sending out information about future
continuing education and for analysis of the changing
SRC profile and demographics. An SRC delegate may
request to see what personal information Optometrists
Association Victoria holds about them at any time.
REFUND POLICY:
1. Please note that refunds will not be processed after Friday 27th April 2012.
2. An Administration fee of $40 applies for cancellation.
NOTICES: All amounts are in Australian dollars and inclusive of GST. The symbol ' ' will be used to denote mandatory fields - these fields must be completed to proceed through the registration process.
CLOSE OF PRE-CONFERENCE REGISTRATION: Please note that pre-conference registrations will close at midnight 17th May 2012.
